A logic model (or logical framework) is a tool to help organize outcomes and objectives with the necessary resources and activities for the successful management of public health problems. A basic framework has been included in your course materials and must be used as a template to map out your practicum.

Complete the attached Logic Model Template using the goals and objectives you have developed with the aid of your classmates and preceptor. Then, provide an account of the resources at your disposal as well as anticipated activities and participation you have planned. You may want to request additional input during your telephone call to your instructor this module/week prior to completing the assignment. The logic model must link student activities and objectives with more long-term outcomes and health impacts as defined by community and/or national public health goals.
